What to Have With You
The forms ask for your medical history and your dental insurance details, so it helps to have your plan card to hand. We are accepting new patients from Shreveport, Bossier, Haughton, Stonewall, Keithville and Doyline.

We accept most PPO plans, see fee-for-service patients, and offer an in-house membership plan.
